Direct Deposit Authorization Form
This form is used to have your paycheck directly deposited into your bank account. You may have the net amount of your paycheck go into a single account or you may have. If you choose to have multiple accounts, you may designate a flat dollar amount into one account and the net amount going into another account.
Please note that it takes one payroll cycle to prenote (set up) your direct deposit. For example, if you turn in your paperwork before the September payroll runs, your direct deposit will go into your account starting with the October paycheck.
W-4 Employee Withholding Allowance Certificate
The IRS W-4 form is used to indicate the number of exemptions you claim for the purpose of calculating your withholding for federal taxes. If you are claiming a different amount for state taxes you must use the state withholding form DE-4.
Tax Sheltered Annuity (TSA)
