• New Speech/Language Referrals
     
    If your child is 2 years, 9 months of age or older and you have significant concerns in the area of speech/language development, you can request an educationally-based speech/language evaluation by submitting a signed, written letter of request to:

    *Please note we do not accept emails as requests/referrals.
     
    In your letter, please include the following:                                                                                                  
    (I) Your child's name and DOB
    (II) Specific concerns/reason for referral
    (III) Your home address, best contact phone number, and email address
    (IV) Information regarding any private/outside speech-language assessment your child has undergone 
     
    You must also enclose two (2) proofs of residency, one of which must be a copy of the deed to your house or current lease agreement. Other forms accepted include SDG&E bills, water bills, etc. Please note we do not accept telephone bills.
     
    Once the district has received your written request for evaluation and proofs of residency, one of the preschool SLPs will contact you within fifteen (15) days to gather background information and/or to develop an Evaluation Plan. At this time, preschool SLPs can also answer any questions you may have regarding the assessment process.
